The Doc class functions as a support specialist and area control provider in Extreme and Insane difficulties through healing capabilities and crowd control effects. Your main job is to keep the team healthy while also giving them smart slowing effects that help them stay alive.

This guide talks about the different ways to build Doc and how to use it so that it works well in different task types and situations.
Core Perks
Suppression Station slows enemies. Peace of Mind increases the slow radius by 20%. Enhanced Reach I, II, and III increase the radius further to a total of 58%.
Doctor's Orders gives each member higher than 75% health 5% damage, for a total of 15%. Strength In Numbers gives each nearby teammate 5% damage. Support System makes you heal 20% more, teammates near you get 15% all ability recharge speed.

Readiness increases reload by 20%. Charitable Glow increases your damage by 15%. Overdose makes you run faster and last longer. Longer cooldown, but it's an ability you don't use that much, so it's a trade-off.
Optional Builds
If you are confident in your shooting skills, go with pistol perks and use the Type 78 Burst Pistol. This weapon has damage comparable to that of the best shotgun if you score weak point hits up close, and can easily deal as much damage as a Gunner or even Demo.
Technician Handgun Training provides 15% accuracy and weak point damage. Phalanx Handgun Expertise gives a 15% rate of fire and accuracy. Phalanx Handgun Mastery provides 15% weak point damage and magazine size.
If you don't prefer damage, go with defense:
Bedside Manner makes you revive twice as fast and bleed out time 25% longer. Rapid Recovery gives more health when you revive them, and bleed out time is another 25% longer.

For other slots, Doc is not a very useful class (very useful in specific missions), so skill slots are not that valuable:
Anesthesiologist provides a 10% current health as a shield. Note that the shield will not recover if it's depleted by friendly fire. It would only recharge if your health is damaged from an enemy attack.
Surgeon's Hands makes you immune to interruptions during interactions. Enhanced Reach IV provides an even bigger slow radius. Fitness Training increases max health and the amount of heal you receive.
Gameplay Tips
Trauma Station's purpose is not to heal but to slow enemies. Do not be discouraged from deploying it for the sake of conserving healing charges. Even a station with 0 healing charge left can still slow enemies. This slow is stackable with other crowd control effects.
You shouldn't ignore its healing completely. If combat is not too intense and your team can afford to care for details, let two team members heal using the station and have the other member, preferably the most injured one, avoid being healed by the station, and heal them with a medkit when health drops low enough.

The most important time to use Combat Stims is when team members are on the move with enemies around, such as moving towards mission objectives during combat, disengaging from enemies when being overwhelmed, leading away enemies, and running back to revive downed teammates.
Missions that are great with Doc
Mission 2-3: The level where Doc shines for their ability to heal. You can tank Synthetic with the Anesthesiologist perk. Just take cover quickly to regain your shield once it's depleted.
DLC-3: Another level in dire need of extra heal. If your team is experienced with that mission and well-coordinated, you can potentially use Doc's Combat Stim to rush through the first maze.
Any mission with a double health card. All missions above work better with an Anesthesiologist equipped.
