Go to Mount Balaur

Head to Mount Balaur, west of Parlita, and enter the cave associated with the Beast of Balaur quest. Visit at night, as you need Coen's vampiric traversal ability to reach the upper section.

Defeat the Beast of Balaur

Make your way through the lower part of the cave and defeat the Beast of Balaur.

After defeating it, investigate the area and then look toward the raised ledge at the back of the arena. You do not get the Vial and Gladius from the Beast itself.

Use Shadowstep to Reach the Hideout

Switch to Coen's vampiric form and use Shadowstep to climb the wall and reach the upper ledge.

Follow the passage into Brencis's old hideout. This area contains both the Gladius and the Phial of Brencis's Blood.

Get Brencis's Blood

Inside the hideout, look for the table beside the central altar.

The Phial of Brencis's Blood is sitting on the table. Loot it to add it to your inventory.

The nearby chest contains the Gladius, so loot it before leaving.

Final Thoughts

The Mount Balaur version of Brencis's Blood is easy to miss because it is in the hidden upper section of the cave rather than the Beast's arena. Defeat the Beast, use Shadowstep to reach the upper hideout at night, then loot the table beside the altar. While you're there, open the nearby chest to get the Gladius.
