Dr. Fetus is the last boss of Super Meat Boy 3D, waiting at the end of Visceraville after you've cleared every world before it. He has four phases, each with its own set of patterns to read. The fight is about positioning and timing, not just fast reactions.
If you've made it this far, the levels leading up to this fight were harder than the boss itself, so the skills are already there. This guide covers every phase so nothing comes as a surprise.

Phase 1 – Cross Lasers And Spinning
Dr. Fetus opens the fight by firing two lasers from his giant robo-hands, forming a cross shape. The hands slide back and forth along the sides of the roof.
The way to handle this is to treat it like a jump rope. Move between the top-right and bottom-left areas of the roof space, jumping each time a laser passes through where you're standing. There's a rhythm to it once you find the timing.
After enough jumps, the cross shape starts rotating counterclockwise while keeping its form. When that happens, stop jumping. Just follow the open space along the ground and move with the rotation. The phase ends when the fists slam down and send you into the next phase.

Phase 2 – Sweeping Hands And Shockwaves
Two sets of hands sweep across the floor from opposite sides. They do this twice. If the platforming up to this point hasn't been a problem, these sweeps won't be either. Stay moving and jump at the right moment as they come through.
After the sweeps, the fists start punching the ground one after the other, sending shockwaves rolling across the floor. You need to jump over them. The key thing to know here is that you can clear more than one shockwave in a single jump when they stack close together. The timing is the hardest part of the whole fight. Watch for the moments when multiple shockwaves line up and clear them all at once instead of jumping each one separately.

Phase 3 – Electrified Floor And Wall Hopping
You drop down to a lower arena for this phase. The moment you land, get onto the left wall. Don't wait. The floor turns electrified in alternating grid patterns that shift between sides, and standing on the ground will kill you.
From the left wall, hop back and forth between the left and right walls as each grid pattern changes. Three wall swaps get you through it. After the third one, the floor goes back to normal. Drop down, and the hands carry you back up to the roof for the final phase.

Phase 4 – Laser Gun And The Final Sprint
Back on the roof, Dr. Fetus brings out a laser gun. He always swings it to the left first, so stand on the right side of the roof. That gives you the most time to react. When the laser swings back toward you, jump over it the same way you handled the lasers in Phase 1.
Once you survive the laser, a large metal ship connects to the back of the roof. Use the boost arrows on the right side to get on top of it. From there, run straight at Dr. Fetus and jump into him. That's the end of the fight. Don't slow down or look for something else to dodge. Full speed, straight line, jump at him, and it's over.
After the fight, a short cutscene plays, followed by a quick escape sequence with Bandage Girl. It's nothing compared to what you just cleared, and the credits are right after it.
